The Impact of Nursing Informatics on Patient Outcomes and Patient Care Efficiencies Planning and implementing change in nursing practice can be challenging. Currently, at this healthcare organization, nurses are performing handoff shift report at the nurse’s station. Research is showing the implementation of bedside nursing report can lead to a decrease in errors and better patient outcomes. The goals of implementing bedside nursing reporting are providing staff with a standardized approach to increase communication between staff at shift change, to involve patients in their treatment, to decrease adverse events, and medical errors. This project was developed to improve the change of shift reporting process on the units to ensure patient safety and patient outcomes are at the center of the hospital’s priorities.

Stakeholders Impacted by Project

Hospital leaders and healthcare organizations are making conscious efforts to change their environments to assure increases in patient safety and satisfaction is a priority (McAllen, Stephen, Swanson-Bieraman, Kerr, & Whiteman, 2018). The Board of Trustees and patients are the primary stakeholders implementing bedside nursing report will impact. It is up to the Board of Trustees to make decisions in the best interests of the hospital. The hospital relies heavily on the expertise of these members to prevent the facility from experiencing catastrophic events.

Medication errors and adverse events affect the hospital negatively. The costs to correct adverse events and errors have the potential to close hospital doors permanently. The patients are the direct consumers of any hospital. Implementing new projects will automatically affect patients who receive services from this hospital. Without patients, the hospital would not be able to prosper financially. If the patients are not receiving adequate care, patients can choose to visit other facilities to receive their needed care. Fewer patients equal less revenue coming into the hospital.

Patient Outcomes or Patient Care Efficiencies Project Will Improve

“Nurse shift changes require the successful transfer of information between nurses to prevent adverse events and medical errors. Patients and families can play a role to make sure these transitions in care are safe and effective. Research shows that when patients are engaged in their health care, it can lead to measurable improvements in safety and quality” (Nurse Bedside Shift Report, 2017). The benefits of bedside reporting are numerous and include increasing patient involvement and understanding of care, dialogue between the patient and nurse on updates on their conditions, increasing accountability of nurses, increasing teamwork and relationships among nurses, increasing patient satisfaction, and decreasing the potential for mistakes to occur (Rush, 2012). Bedside report begins with the outgoing nurse introducing the oncoming nurse to the patient, followed by an assessment of the patient and environment. The patient assessment will include a general overview of the patient’s condition and significant aspects of care. These conditions can range from wound sites to dressings, abnormal breath or heart sounds, surveying the room for safety, assessing the intravenous site for inflammation and fluids, or anything considered out of the ordinary (McAllen et al., 2018). The communication during the bedside shift report is intended to ensure the continuum of care and patient safety.

Bedside nursing report promotes clear and open communication between the nurses and the patient and therefore decreases the potential for errors (McAllen et al., 2018).

Technology Required to Implement Project

Since the earliest days of technology adoption in health care, nursing informatics professionals have been at the forefront of leading change (Sugrue, 2013). Technology enables care and enhances safety by simplifying functions that could be considered both simple and complex tasks. Electronic Health Records (EHRs) will play an integral role in implementing bedside nursing report. Nurses are responsible for the majority of EHR documentation during their shifts. Some of these responsibilities include developing plans of care, physiological parameters, assessments, interventions, and progress evaluations, which are critical to care integration and patient safety (Glassman, 2017). Also, the development of a computerized system to track errors discovered by bedside nursing report. “Clinical transformation involves assessing and continually improving the way patient care is delivered at all levels in a care delivery organization. It occurs when an organization rejects existing practice patterns that deliver inefficient or less effective results and embraces a common goal of patient safety, clinical outcomes, and quality care through process redesign and IT implementation” (Sugrue, 2013, p. 18). For example, medication error reporting or patient falls can be issues discovered and mitigated during bedside nursing report. This system would need to be in place to analyze, measure, and evaluate data to continue the needed improvements to patient safety gained with bedside nursing report (McAllen et al., 2018).

Members of the Project Team

The project team will consist of the Quality Improvement Manager. Initially, the Quality Improvement Manager will need to meet with the hospital leaders to discuss the budget and financial backings of the project. Ultimately, the budget will determine if a new project is financially doable for the hospital at this time. The Quality Improvement Manager will be responsible for designing and implementing the new approach to receiving and obtaining bedside report at shift change. He or she will also provide and explain the new changes to the nurse managers of every hospital unit. The nurse managers will play instrumental roles as members of the project team. The nurse managers will be responsible for implementing bedside nursing report on their units. They are also responsible for providing the nurses on the units with the proper training to ensure the implementation of beside report is a smooth transition. The facility may also enforce a time-sensitive deadline for the project to be an active process on every hospital unit. The nurse manager will be responsible for meeting this deadline on their units. “Nurse informaticists must facilitate the inclusion of computational tools and algorithms to help handle the collection, organization, analysis, processing, presentation, and dissemination of biological data” (McGonigle & Mastrian, 2018, p. 546). The nurse informaticist would create a system to track, measure, and evaluate the types of errors found during bedside report. The tracking of this data would be beneficial to provide further improvements to patient safety gained with bedside report (Surgue, 2013).

Conclusion

Planning and implementing change in nursing practice can present challenges. Patient care and how the care is delivered is the foundation in which healthcare organizations originated. Without a doubt, technology enhancing tools affect every aspect of the nursing process in every care delivery environment. To increase patient satisfaction and safety, healthcare facilities must be committed to providing seamless care effectively and efficiently. Healthcare organizations can do this more effectively through such nurse communication processes as bedside nursing report. Implementing the bedside nursing report processes into the nurse workflow through technology that builds trust and relationships and creates connectivity with patients outside the hospital walls. Closing the communication gaps through bedside nursing report that separate nurses, patients, and families improve patient and staff satisfaction and clinical outcomes while bringing financial benefits to the organization (Collins, 2014).
